How do I become a Medical Marijuana patient?

To begin the application process, you will need to register for the Medical Marijuana Program and set up a profile in the Patient and Caregiver. Click here to be directed to the Department of Health’s website to register.

You must be a resident of Pennsylvania with proof of residency from a valid PA driver’s license or state-issued ID with your current address. If you meet these requirements, you can register for the program.

After you register, complete an in-person examination with a registered physician in the Medical Marijuana Program to confirm that you suffer from one of the 23 qualified medical conditions.

Once approved by a registered physician, you will receive a certificate from your physician. The final step is to pay a $50 processing fee for the medical marijuana ID card. After receiving your ID card, you can use it to purchase medical marijuana from an approved Department of Health medical marijuana dispensary in Pennsylvania.

How do I become a Caregiver for a patient?

Visit the Pennsylvania Department of Health’s Patient and Caregiver Registry at medicalmarijuana.pa.gov and choose the form to fill out – caring for an adult patient, or caring for a minor or person with a disability. (Click the link above to be directed to the website).

Complete a criminal background check through the Pennsylvania State Police. Based on the results, the Department of Health determines the applicant’s “character, fitness and suitability” for the task. Applicants convicted within the previous five years of selling or possessing drugs, narcotics or controlled substances can be rejected. Grounds for denial also include a history of drug abuse or diverting drugs.

Complete an authorization form designating the third-party caregiver, if necessary.

Once accepted, applicants return to the Patient and Caregiver Registry and pay the $50 fee for a medical marijuana card. Fees can be waived or reduced for applicants who document financial hardship.
